CONFIRM YOUR APPOINTMENT
We will call to confirm your appointment two days prior. If we cannot reach you, we leave a message. Please call back to confirm your scheduled appointment at 901.624.3333.
YOUR OFFICE VISIT
What to bring for your appointment:
- Insurance card
- Patient Information and Health History Forms
- Physician referral – If you have a referral prior to your appointment, please bring your referral form. Appointments regarding cosmetic and aesthetic concerns cannot generally be billed to insurance and referrals are not necessary.
- Payment* for your co-pay, if your insurance plan requires one.
*Note: We gladly accept cash, all major credit cards, and debit cards. We do not accept personal checks.
If you are a new patient, please arrive 15 minutes before your first visit to fill out any paperwork.
INSURANCE & BILLING
We take most of the major insurance plans at Levy Dermatology, P.C. Please remember that insurance is considered a method of reimbursing the patient for fees paid to Levy Dermatology, P.C. and is not substitute for payment. As a courtesy to our patients, we will verify your insurance benefits for you and, if possible, notify you of those benefits prior to or at the time of your visit. Many insurance policies have co-pays, co-insurance payments, and deductibles. Some companies pay fixed allowances for certain procedures and others pay a percentage of the charges. In order to control the cost of billings, we request that any amount owed from your visit be paid at the day of service. Please double check with your insurance company to verify your benefits prior to your visit.
Prior to your medical appointment please have your referral information sent to our office.
Cosmetic treatments are an out-of-pocket expense not covered by insurance, and they may require a prior consultation with the physician. The consultation will incur a nominal fee. This fee will be applied to the cost of the treatment as long as it is performed within one year of your consultation.
NO SHOW POLICY
Out of respect for other patients and our doctors’ time, kindly give us the courtesy of cancelling prior to 24 of your appointment. Failure to do so will incur a $35 cancellation/no-show fee to your account for a medical or surgical appointment and $80 for any cosmetic treatment (due to the larger time allotted for these procedures). We appreciate your understanding and cooperation with this policy.
