Your first visit to Levy Dermatology involves a few special steps so that we can get to know you. For a seamless welcome to our practice, please read through this website. We recommend you save some time by printing out and completing the patient forms in advance of your appointment.

- A. Insurance Card- needed to schedule your appointment.
- B. Primary Physician- the name of your primary physician
- C. Referring Physician- the name of the physician who referred you here (if applicable)

- We will call two days prior to your appointment to confirm your appointment. If we cannot reach you, we leave a message. Please call back to confirm your scheduled appointment. (901) 624-3333.
Your Office Visit
What to bring for your appointment:
- Insurance card
- Patient Information and Health History Forms
- Physician referral - If you have a referral prior to your appointment, please bring your referral form. Appointments concerning cosmetic and aesthetic concerns cannot generally be billed to insurance and referrals are not necessary.
- Payment* for your co-pay, if your insurance plan requires one.
*Note: We gladly accept cash, all major credit cards, and debit cards. We do not accept personal checks.
If you are a new patient please arrive 15 minutes before your first visit to fill out any paperwork.

Insurance & Billing
Prior to your medical appointment please have your referral information sent to our office.
Cosmetic treatments are an out-of-pocket, non-insurance covered expense and require a prior consultation with the physician. A consult fee of $80 will be charged and applied to the cost of the procedure if performed within one year of consultation.
